In order to access your myHealthPointe account with this option, you must have a mobile phone number on file. Please enter your name, date of birth, and mobile number to look up your record.
Note: Clients under the age of 18 or phone numbers that are tied to multiple accounts must use the Sign in with Credentials option. If you do not have credentials, please contact your provider to assist you with registering your myHealthPointe account.
